Will Deacon a2c42bbabb arm64: spectre: Prevent lockdep splat on v4 mitigation enable path
The Spectre-v4 workaround is re-configured when resuming from suspend,
as the firmware may have re-enabled the mitigation despite the user
previously asking for it to be disabled.

Enabling or disabling the workaround can result in an undefined
instruction exception on CPUs which implement PSTATE.SSBS but only allow
it to be configured by adjusting the SPSR on exception return. We handle
this by installing an 'undef hook' which effectively emulates the access.

Installing this hook requires us to take a couple of spinlocks both to
avoid corrupting the internal list of hooks but also to ensure that we
don't run into an unhandled exception. Unfortunately, when resuming from
suspend, we haven't yet called rcu_idle_exit() and so lockdep gets angry
about "suspicious RCU usage". In doing so, it tries to print a warning,
which leads it to get even more suspicious, this time about itself:

Commit f9ce0be71d ("mm: Cleanup faultaround and finish_fault()
codepaths") added a call to 'update_mmu_cache()' in mm/filemap.c, which
breaks the build for microblaze:

Will Deacon 8cf55f24ce x86/ldt: Use tlb_gather_mmu_fullmm() when freeing LDT page-tables
free_ldt_pgtables() uses the MMU gather API for batching TLB flushes
over the call to free_pgd_range(). However, tlb_gather_mmu() expects
to operate on user addresses and so passing LDT_{BASE,END}_ADDR will
confuse the range setting logic in __tlb_adjust_range(), causing the
gather to identify a range starting at TASK_SIZE. Such a large range
will be converted into a 'fullmm' flush by the low-level invalidation
code, so change the caller to invoke tlb_gather_mmu_fullmm() directly.

Will Deacon d8b450530b tlb: mmu_gather: Introduce tlb_gather_mmu_fullmm()
Passing the range '0, -1' to tlb_gather_mmu() sets the 'fullmm' flag,
which indicates that the mm_struct being operated on is going away. In
this case, some architectures (such as arm64) can elide TLB invalidation
by ensuring that the TLB tag (ASID) associated with this mm is not
immediately reclaimed. Although this behaviour is documented in
asm-generic/tlb.h, it's subtle and easily missed.

Introduce tlb_gather_mmu_fullmm() to make it clearer that this is for the
entire mm and WARN() if tlb_gather_mmu() is called with the 'fullmm'
address range.

invalidation.

Will Deacon 912efa17e5 mm: proc: Invalidate TLB after clearing soft-dirty page state
Since commit 0758cd8304 ("asm-generic/tlb: avoid potential double
flush"), TLB invalidation is elided in tlb_finish_mmu() if no entries
were batched via the tlb_remove_*() functions. Consequently, the
page-table modifications performed by clear_refs_write() in response to
a write to /proc/<pid>/clear_refs do not perform TLB invalidation.
Although this is fine when simply aging the ptes, in the case of
clearing the "soft-dirty" state we can end up with entries where
pte_write() is false, yet a writable mapping remains in the TLB.

Fix this by avoiding the mmu_gather API altogether: managing both the
'tlb_flush_pending' flag on the 'mm_struct' and explicit TLB
invalidation for the sort-dirty path, much like mprotect() does already.

a bunch of apparently "unused" header inclusions from our mm/mmap.c
implementation, but in doing so introduced the following warning when

Will Deacon 5857c9209c mm: Mark anonymous struct field of 'struct vm_fault' as 'const'
The fields of this struct are only ever read after being initialised, so
mark it 'const' before somebody tries to modify it again. GCC will then
complain (with an error) about modification of these fields after they
have been initialised, although LLVM currently allows them without even
a warning:

Will Deacon 9d3af4b448 mm: Pass 'address' to map to do_set_pte() and drop FAULT_FLAG_PREFAULT
Rather than modifying the 'address' field of the 'struct vm_fault'
passed to do_set_pte(), leave that to identify the real faulting address
and pass in the virtual address to be mapped by the new pte as a
separate argument.

This makes FAULT_FLAG_PREFAULT redundant, as a prefault entry can be
identified simply by comparing the new address parameter with the
faulting address, so remove the redundant flag at the same time.

'struct vm_fault' contains both information about the fault being
serviced alongside mutable fields contributing to the state of the
fault-handling logic. Unfortunately, the distinction between the two is
not clear-cut, and a number of callers end up manipulating the structure
temporarily before restoring it when returning.

Try to clean this up by moving the immutable fault information into an
anonymous struct, which will later be marked as 'const'. Ideally, the
'flags' field would be part of the new structure too, but it seems as
though the ->page_mkwrite() path is not ready for this yet.

On CPUs with hardware AF/DBM, initialising prefaulted PTEs as 'old'
improves vmscan behaviour and does not appear to introduce any overhead
elsewhere.

Implement arch_wants_old_prefaulted_pte() to return 'true' if we detect
hardware access flag support at runtime. This can be extended in future
based on MIDR matching if necessary.

Will Deacon 46bdb4277f mm: Allow architectures to request 'old' entries when prefaulting
Commit 5c0a85fad9 ("mm: make faultaround produce old ptes") changed
the "faultaround" behaviour to initialise prefaulted PTEs as 'old',
since this avoids vmscan wrongly assuming that they are hot, despite
having never been explicitly accessed by userspace. The change has been
shown to benefit numerous arm64 micro-architectures (with hardware
access flag) running Android, where both application launch latency and
direct reclaim time are significantly reduced (by 10%+ and ~80%
respectively).

Unfortunately, commit 315d09bf30 ("Revert "mm: make faultaround
produce old ptes"") reverted the change due to it being identified as
the cause of a ~6% regression in unixbench on x86. Experiments on a
variety of recent arm64 micro-architectures indicate that unixbench is
not affected by the original commit, which appears to yield a 0-1%
performance improvement.

Since one size does not fit all for the initial state of prefaulted
PTEs, introduce arch_wants_old_prefaulted_pte(), which allows an
architecture to opt-in to 'old' prefaulted PTEs at runtime based on
whatever criteria it may have.

Will Deacon dca5244d2f compiler.h: Raise minimum version of GCC to 5.1 for arm64
GCC versions >= 4.9 and < 5.1 have been shown to emit memory references
beyond the stack pointer, resulting in memory corruption if an interrupt
is taken after the stack pointer has been adjusted but before the
reference has been executed. This leads to subtle, infrequent data
corruption such as the EXT4 problems reported by Russell King at the
link below.

Life is too short for buggy compilers, so raise the minimum GCC version
required by arm64 to 5.1.

lockup_detector_init() makes heavy use of per-cpu variables and must be
called with preemption disabled. Usually, it's handled early during boot
in kernel_init_freeable(), before SMP has been initialised.

Since we do not know whether or not our PMU interrupt can be signalled
as an NMI until considerably later in the boot process, the Arm PMU
driver attempts to re-initialise the lockup detector off the back of a
device_initcall(). Unfortunately, this is called from preemptible

Handling all combinations of the VMAP_STACK and SHADOW_CALL_STACK options
in sdei_arch_get_entry_point() makes the code difficult to read,

Will Deacon ff1712f953 arm64: pgtable: Ensure dirty bit is preserved across pte_wrprotect()
With hardware dirty bit management, calling pte_wrprotect() on a writable,
dirty PTE will lose the dirty state and return a read-only, clean entry.

Move the logic from ptep_set_wrprotect() into pte_wrprotect() to ensure that
the dirty bit is preserved for writable entries, as this is required for
soft-dirty bit management if we enable it in the future.

Will Deacon 07509e10dc arm64: pgtable: Fix pte_accessible()
pte_accessible() is used by ptep_clear_flush() to figure out whether TLB
invalidation is necessary when unmapping pages for reclaim. Although our
implementation is correct according to the architecture, returning true
only for valid, young ptes in the absence of racing page-table
modifications, this is in fact flawed due to lazy invalidation of old
ptes in ptep_clear_flush_young() where we elide the expensive DSB
instruction for completing the TLB invalidation.

Rather than penalise the aging path, adjust pte_accessible() to return
true for any valid pte, even if the access flag is cleared.

The EL2 vectors installed when a guest is running point at one of the
following configurations for a given CPU:

  - Straight at __kvm_hyp_vector
  - A trampoline containing an SMC sequence to mitigate Spectre-v2 and
    then a direct branch to __kvm_hyp_vector
  - A dynamically-allocated trampoline which has an indirect branch to
    __kvm_hyp_vector
  - A dynamically-allocated trampoline containing an SMC sequence to
    mitigate Spectre-v2 and then an indirect branch to __kvm_hyp_vector

The indirect branches mean that VA randomization at EL2 isn't trivially
bypassable using Spectre-v3a (where the vector base is readable by the
guest).

Rather than populate these vectors dynamically, configure everything
statically and use an enumerated type to identify the vector "slot"
corresponding to one of the configurations above. This both simplifies
the code, but also makes it much easier to implement at EL2 later on.

Will Deacon da592e68a5 KVM: arm64: Re-jig logic when patching hardened hyp vectors
The hardened hyp vectors are not used on systems running with VHE or CPUs
without the ARM64_HARDEN_EL2_VECTORS capability.

Re-jig the checking logic slightly in kvm_patch_vector_branch() so that
it's a bit clearer what we're looking for. This is purely cosmetic.

Will Deacon 04e613ded8 arm64: smp: Tell RCU about CPUs that fail to come online
Commit ce3d31ad3c ("arm64/smp: Move rcu_cpu_starting() earlier") ensured
that RCU is informed early about incoming CPUs that might end up calling
into printk() before they are online. However, if such a CPU fails the
early CPU feature compatibility checks in check_local_cpu_capabilities(),
then it will be powered off or parked without informing RCU, leading to
an endless stream of stalls:

Will Deacon f969f03888 arm64: errata: Fix handling of 1418040 with late CPU onlining
In a surprising turn of events, it transpires that CPU capabilities
configured as ARM64_CPUCAP_WEAK_LOCAL_CPU_FEATURE are never set as the
result of late-onlining. Therefore our handling of erratum 1418040 does
not get activated if it is not required by any of the boot CPUs, even
though we allow late-onlining of an affected CPU.

In order to get things working again, replace the cpus_have_const_cap()
invocation with an explicit check for the current CPU using
this_cpu_has_cap().

Will Deacon e35123d83e arm64: lto: Strengthen READ_ONCE() to acquire when CONFIG_LTO=y
When building with LTO, there is an increased risk of the compiler
converting an address dependency headed by a READ_ONCE() invocation
into a control dependency and consequently allowing for harmful
reordering by the CPU.

Ensure that such transformations are harmless by overriding the generic
READ_ONCE() definition with one that provides acquire semantics when
building with LTO.

Will Deacon 364a5a8ae8 arm64: cpufeatures: Add capability for LDAPR instruction
Armv8.3 introduced the LDAPR instruction, which provides weaker memory
ordering semantics than LDARi (RCpc vs RCsc). Generally, we provide an
RCsc implementation when implementing the Linux memory model, but LDAPR
can be used as a useful alternative to dependency ordering, particularly
when the compiler is capable of breaking the dependencies.

Since LDAPR is not available on all CPUs, add a cpufeature to detect it at
runtime and allow the instruction to be used with alternative code
patching.

Will Deacon e2fc6a9f68 KVM: arm64: Fix masks in stage2_pte_cacheable()
stage2_pte_cacheable() tries to figure out whether the mapping installed
in its 'pte' parameter is cacheable or not. Unfortunately, it fails
miserably because it extracts the memory attributes from the entry using
FIELD_GET(), which returns the attributes shifted down to bit 0, but then
compares this with the unshifted value generated by the PAGE_S2_MEMATTR()
macro.

A direct consequence of this bug is that cache maintenance is silently
skipped, which in turn causes 32-bit guests to crash early on when their
set/way maintenance is trapped but not emulated correctly.

Fix the broken masks by avoiding the use of FIELD_GET() altogether.

Will Deacon ea8f8c99a2 arm64: spectre-v2: Favour CPU-specific mitigation at EL2
Spectre-v2 can be mitigated on Falkor CPUs either by calling into
firmware or by issuing a magic, CPU-specific sequence of branches.
Although the latter is faster, the size of the code sequence means that
it cannot be used in the EL2 vectors, and so there is a need for both
mitigations to co-exist in order to achieve optimal performance.

Change the mitigation selection logic for Spectre-v2 so that the
CPU-specific mitigation is used only when the firmware mitigation is
also available, rather than when a firmware mitigation is unavailable.

Qian Cai reports that TX2 no longer boots with his .config as it appears
that task_cpu() gets instrumented and used before KASAN has been
initialised.

Although Mark has a proposed fix, let's take the safe option of reverting
this for now and sorting it out properly later.

Will Deacon ffd1b63a58 KVM: arm64: Ensure user_mem_abort() return value is initialised
If a change in the MMU notifier sequence number forces user_mem_abort()
to return early when attempting to handle a stage-2 fault, we return
uninitialised stack to kvm_handle_guest_abort(), which could potentially
result in the injection of an external abort into the guest or a spurious
return to userspace. Neither or these are what we want to do.

Initialise 'ret' to 0 in user_mem_abort() so that bailing due to a
change in the MMU notrifier sequence number is treated as though the
fault was handled.

Will Deacon b259d137e9 KVM: arm64: Pass level hint to TLBI during stage-2 permission fault
Alex pointed out that we don't pass a level hint to the TLBI instruction
when handling a stage-2 permission fault, even though the walker does
at some point have the level information in its hands.

Rework stage2_update_leaf_attrs() so that it can optionally return the
level of the updated pte to its caller, which can in turn be used to
provide the correct TLBI level hint.

Will Deacon 80d6b46667 arm64: dbm: Invalidate local TLB when setting TCR_EL1.HD
TCR_EL1.HD is permitted to be cached in a TLB, so invalidate the local
TLB after setting the bit when detected support for the feature. Although
this isn't strictly necessary, since we can happily operate with the bit
effectively clear, the current code uses an ISB in a half-hearted attempt
to make the change effective, so let's just fix that up.

Will Deacon 6a1bdb173f arm64: mm: Make flush_tlb_fix_spurious_fault() a no-op
Our use of broadcast TLB maintenance means that spurious page-faults
that have been handled already by another CPU do not require additional
TLB maintenance.

Make flush_tlb_fix_spurious_fault() a no-op and rely on the existing TLB
invalidation instead. Add an explicit flush_tlb_page() when making a page
dirty, as the TLB is permitted to cache the old read-only entry.

Will Deacon 780c083a8f arm64: Add support for PR_SPEC_DISABLE_NOEXEC prctl() option
The PR_SPEC_DISABLE_NOEXEC option to the PR_SPEC_STORE_BYPASS prctl()
allows the SSB mitigation to be enabled only until the next execve(),
at which point the state will revert back to PR_SPEC_ENABLE and the
mitigation will be disabled.

Add support for PR_SPEC_DISABLE_NOEXEC on arm64.

Will Deacon 9ef2b48be9 KVM: arm64: Allow patching EL2 vectors even with KASLR is not enabled
Patching the EL2 exception vectors is integral to the Spectre-v2
workaround, where it can be necessary to execute CPU-specific sequences
to nobble the branch predictor before running the hypervisor text proper.

Remove the dependency on CONFIG_RANDOMIZE_BASE and allow the EL2 vectors
to be patched even when KASLR is not enabled.

Will Deacon 532d581583 arm64: Treat SSBS as a non-strict system feature
If all CPUs discovered during boot have SSBS, then spectre-v4 will be
considered to be "mitigated". However, we still allow late CPUs without
SSBS to be onlined, albeit with a "SANITY CHECK" warning. This is
problematic for userspace because it means that the system can quietly
transition to "Vulnerable" at runtime.

Avoid this by treating SSBS as a non-strict system feature: if all of
the CPUs discovered during boot have SSBS, then late arriving secondaries
better have it as well.

The Spectre-v2 mitigation code is pretty unwieldy and hard to maintain.
This is largely due to it being written hastily, without much clue as to
how things would pan out, and also because it ends up mixing policy and
state in such a way that it is very difficult to figure out what's going
on.

Rewrite the Spectre-v2 mitigation so that it clearly separates state from
policy and follows a more structured approach to handling the mitigation.

Will Deacon 6e5f092784 arm64: Remove Spectre-related CONFIG_* options
The spectre mitigations are too configurable for their own good, leading
to confusing logic trying to figure out when we should mitigate and when
we shouldn't. Although the plethora of command-line options need to stick
around for backwards compatibility, the default-on CONFIG options that
depend on EXPERT can be dropped, as the mitigations only do anything if
the system is vulnerable, a mitigation is available and the command-line
hasn't disabled it.

Remove CONFIG_HARDEN_BRANCH_PREDICTOR and CONFIG_ARM64_SSBD in favour of
enabling this code unconditionally.
