Drop the bogus endpoint-lookup helper which could end up accepting
interfaces based on endpoints belonging to unrelated altsettings.
Note that the returned bulk pipes and interrupt endpoint descriptor
were never actually used. Instead the bulk-endpoint numbers are
hardcoded to 1 and 2 (matching the specification), while the interrupt-
endpoint descriptor was assumed to be the third descriptor created by
USB core.
Try to bring some order to this by dropping the bogus lookup helper and
adding the missing endpoint sanity checks while keeping the interrupt-
descriptor assumption for now.

The set-led command is eight bytes long and starts with a command byte
followed by six bytes of RGB data and ends with a byte encoding a

The led activity helpers had a few long-standing bugs which corrupted
the command packets by inserting a second command byte and thereby
offsetting the RGB data and dropping the frequency in non-xmas mode.
In xmas mode, a related off-by-one error left the frequency field

Note that the break flag can be left set also for a packet received

Commit a8d78d9f38 ("USB: serial: clean up throttle handling")
converted the throttle handling to use atomic bitops. This means that we
can relax the smp_mb() in unthrottle() to smp_mb__after_atomic(), which
for example is a no-op on architectures like x86 that provide fully
ordered atomics.

While assumed not to make a difference, not using the factor-2 prescaler
makes the receiver more susceptible to errors.
Specifically, there have been reports of problems with devices that
cannot generate a 115200 rate with a smaller error than 2.1% (e.g.
117647 bps). But this can also be reproduced with a low-speed RS232
tranceiver at 115200 when the input rate matches the nominal rate.
So whenever possible, enable the factor-2 prescaler and halve the
divisor in order to use settings closer to that of the previous
algorithm.

USB-serial drivers must not be unbound from their ports before the
corresponding USB driver is unbound from the parent interface so
suppress the bind and unbind attributes.
Unbinding a serial driver while it's port is open is a sure way to
trigger a crash as any driver state is released on unbind while port
hangup is handled on the parent USB interface level. Drivers for
multiport devices where ports share a resource such as an interrupt
endpoint also generally cannot handle individual ports going away.

Amend the endpoint-descriptor sanity checks to detect all duplicate
endpoint addresses in a configuration.
Commit 0a8fd13462 ("USB: fix problems with duplicate endpoint
addresses") added a check for duplicate endpoint addresses within a
single alternate setting, but did not look for duplicate addresses in
other interfaces.
The current check would also not detect all duplicate addresses when one
endpoint is as a (bi-directional) control endpoint.
This specifically avoids overwriting the endpoint entries in struct
usb_device when enabling a duplicate endpoint, something which could
potentially lead to crashes or leaks, for example, when endpoints are
later disabled.
